We read with great interest the Platinum Priority editorial by Marko Babjuk [1]. The author mentions that for evaluation of the role of systemic neoadjuvant treatment before radical cystectomy (RC), its antitumor efficacy, toxicity, and impact on complications related to surgery should be taken into account. We agree with the author on this topic. We believe, however, that cost is another important factor worth considering.
