We read with interest the article by De Bleser et al [1] comparing outcomes for metastasis-directed treatment (MDT) only, namely stereotactic body radiotherapy (SBRT), versus elective nodal RT (ENRT) for patients with nodal recurrences from prostate cancer. In their study, ENRT reduced the number of lymph node recurrences when compared with SBRT, albeit with higher toxicity. However, we believe that the term ENRT is somewhat misleading and that it should be better described in terms of both target volumes and required dose.
