The EAU-ESTRO-ESUR-SIOG guidelines [1] on prostate cancer (PCa) recommend performing multiparametric magnetic resonance imaging (mpMRI) in men with a prior negative prostate biopsy (PBx; evidence level 1a and strong rating). Moreover, they also recommended performing only targeted PBx (TPBx) for lesions with a Prostate Imaging-Reporting and Data System (PI-RADS) score >=3 (evidence level 2a and weak rating) [1]. The evidence for avoiding systematic PBx (SPBx) comes from a Cochrane meta-analysis [2] that has recently been published in European Urology [3].
